This involves an on-site assessment of the health care systems level of readiness to implement a primary care behavioral health integration program. After meeting with leadership team members, opinion leaders and operational support staff, we conduct a “gap analysis” of where your organization is at and what steps need to be taken to prepare for a successful implementation. I offer a wide variety of written support materials, including a boilerplate “program manual” that defines every aspect of the integrated model of care, care process flow documents, and clinical process support tools. In a major system initiative like this, it is really important to anticipate and plan for every possible detail, knowing full well that unexpected details will appear and have to be resolved as part of the implementation process.
